FORMS
You can use periodic forms to gather data from users with data entry rights and save that data in the BMP database.
A periodic form resembles a "mini spreadsheet". It contains one line for each combination of node or node type data to register, and one column for each time period against which to include the data.
In Configuration Studio, periodic forms are stored in Categories. You must create at least one category to store your forms.
Users data entry rights can access configured Periodic Forms by navigating to My Forms under MY RESPONSIBILITIES on Web.
You create periodic forms in the Forms model as follows:
By default, each form contains a Form page that stores its questions.
To create questions, perform the following:
Corporater BMP supports the following question types:
If don't have a valid license for Forms, the only question type that's available is Multi period question.
To reorganize the tree structure in the Forms model, use the Cut or Copy + Paste functionality to move or copy forms to different categories. You can move and copy a
forms even after you activate it.
General
Name - the name of the periodic form.
The default name is "Periodic Form". However, you can change it to a value of your choice.
Description - an optional text to describe the from.
Responsible - the person who is responsible for the form.
Click the Responsible button() and select one or more users from the list.
Distribution
Start date - the first day the form is available for data entry.
End date - the last day the form is available for data entry.
Storing period - the date against which Corporater BMP stores the form.
Click the Date calculation button () and specify an adjusting function based on the form's distribution date. For example, use '-1M' for the previous month.
Due - the date relative to the distribution date when the form data is due. For example, use '+1W' for one week.
Distribution Lists - the list of users to which Corporater BMP distributes the form.
Click the Distribution button () and create a new or select one or multiple existing distribution lists.
Stay open after submit - select for users to be able to resubmit the form.
If deselected and the form includes a Multi Period question that accepts data for several
periods, Corporater BMP closes the form after the users submit data for all the periods.
Referred respondents - select for the form to reference its respondents.
Start automatically - select for Corporater BMP to start the form automatically.
Corporater BMP can start only valid forms.
If you stop or reset the form, the option becomes deselected again.
Period type - the period type against which Corporater BMP registers the form data.
Information
Auto-generate nodes - select for Corporater BMP to create a node for each question in the form, to store its answer in the database.
Corporater BMP uses the question name as the node name and includes the node's name and ID and in the question's Node field.
If the Name field for a question is empty, Corporater BMP uses the value in the Question text field as the node name.
After Corporater BMP creates the nodes for the form, the option becomes deselected again.
Even if you select the option, you can still specify nodes of your choice. In this case, Corporater BMP no longer creates nodes.
If you select the option and add new questions to the form after its distribution, Corporater BMP automatically creates nodes for the new questions when the form is distributed again.
If you leave the option deselected, you must specify in the configuration of each question and subquestion a node for Corporater BMP to use to store its answer.
Parent node – applies only if you select Auto-generate nodes. The node under which Corporater BMP creates each auto-generated node for the form.
When it auto-generates nodes, Corporater BMP creates a structure with the node you specify as the starting point. For
each question in the form, it creates a new node directly under this node. For each subquestion, it creates a node under the question's node.
Status
Distribution status - the status of the form
Available
statuses are:
Draft - under construction
Pending - awaiting distribution
Started - activated
Stopping - stopped but waiting for respondents to answer
Stopped
Completed
Layout
Page layout - the page layout that Corporater BMP applies to the form page on Web.

Two column layout
Show information bar - select for Corporater BMP to display a horizontal bar with information about the form such as its name, organization, dates, and status at the top of the form on Web.
Save temporarily - select for the corresponding button to be available on the form on Web.
Users can use this button to temporarily save their answers and submit the form later.
Audit
Modified by - the name of the last user to make changes to the object.
Last modified - the date and time when the object was last changed.
Created by - the name of the user who created the object.
Created - the date and time when the object was created.
ID
ID - a unique ID for the object.
By
default, Corporater BMP assigns consecutive numbers to new objects.
You can change the ID to a value of choice that can include either letters or letter and number combinations.
The ID mustn't contain only numbers, include special characters such as ) ( . * } { + < > [ ], or match the name of a default object type in Corporater BMP.
Availability
Periods before - number of time periods before the current one that are open for data input
Periods after - number of time periods after the current one that are open for data input
Access control
Read accessors - click the Edit button () and authorize one or more users and user groups to view the object on Web.
Add accessors - click the Edit button() and authorize one or more users and user groups to add new objects to the object on Web.
Write accessors - click the Edit button() and authorize one or more users and user groups to edit the object on Web.
Delete accessors - click the Edit button() and authorize one or more users and user groups to delete the object on Web.
Ownership - click the Edit button () and grant ownership of the object to one or more users and user groups.
Users and user groups who own the object have full access rights to it.
Override access profile - select for the object's access control setting to outrank the object's corresponding access settings in profiles.
